*POSITION DESCRIPTION *
*Outpatient Medical Center Family Nurse Practitioner *
_Salary Range: based on experience, qualifications, and averages for similar FQHC work in Louisiana _
GENERAL
As a licensed, privileged family nurse practitioner, provide healthcare services to high-quality primary care patients of all ages for a _non-profit organization with FQHC health services in Leesville and/or Natchitoches, La.
_Under the administrative supervision of the CEO/CMO and the clinical supervision of the Medical Director, the nurse practitioner will be responsible for assessing, examining, diagnosing, and treating OMC patients in accordance with established scope of practice, privileging, and standards of care.
The nurse practitioner will be responsible for prescribing patients appropriate medication, health regimens, referrals, and lifestyle adjustments that will contribute to improving their health, wellbeing, and management of illnesses.
Consults supervising physicians as appropriate to assure quality of care.
*Essential Responsibilities: *
Performs thorough patient care examinations to include histories, physical examination, diagnostic testing, and anticipatory counseling.
Order laboratory and radiology tests and refer patients to specialists when appropriate.
Analyze reports, test results, medical records and examinations to diagnose condition of patients and propose treatment options.
Prescribe pharmaceuticals, other medications, and treatment regimens to treat identified and documented medical conditions.
Provides appropriate health education and guidance to include discussing potential side effects of medication/treatment with patients/guardians and ensure that all allergy information is upto-date before prescribing medication/treatment.
Provide advice to patients for lifestyle and diet changes that may improve their health or help to treat the health issue they are experiencing.
Adhere to OMC’s administrative and clinical policies and procedures departmental policies, procedures and objectives; participates in ongoing quality assurance and quality improvement activities to include infection control, peer reviews, etc.
Maintain strict patient confidentiality and comply with all federal and state health laws and regulations.
Records complete, timely and legible medical records; must meet all clinical business requirements including but not limited to, timely completion of charts, review of test results, coding of encounters, attention to preventive health maintenance, following up with patients, etc.
Other appropriate duties, include peer review and QA/QI program activities as assigned by CMO and/or Medical Director.
*Qualifications *
Requires
* Licensed to practice in the state of Louisiana
* DEA and CDS licenses
* Current CPR certification
* Experience providing team-oriented primary healthcare
* Excellent written and oral communication and inter-personal skills.
Preference to providing adult, women’s health and basic pediatrics services.
