Job Overview:
We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team.

Responsibilities:
- Conduct diagnostic evaluations to assess patients' medical conditions and needs
- Collaborate with primary care physicians, specialists, and other healthcare professionals to develop individualized care plans
- Provide direct patient care, including administering medications, monitoring vital signs, and assisting with procedures
- Coordinate and facilitate communication between patients, families, and healthcare providers
- Monitor patient progress and make adjustments to care plans as necessary
- Educate patients and their families on managing their health conditions and promoting wellness
- Advocate for patients' needs and ensure they receive appropriate medical services
- Document all patient interactions, assessments, and interventions accurately and timely
Requirements:
- Valid Registered Nurse (RN) license
- Experience in acute care or critical care settings preferred
- Strong knowledge of physiology and medical terminology
- Excellent communication and interpersonal skills
- Ability to work well in a fast-paced environment
- Strong organizational skills with attention to detail
- Proficiency in using electronic medical records (EMR) systems
- Knowledge of managed care principles and practices
