We are seeking Registered Nurse (RN) or Licensed Practical Nurse (LPN) who will work in collaboration with physicians, dentists, and other health care professionals to provide individuals with intellectual and or developmental disabilities the healthcare needs, education and supports required to reach and or maintain their optimum health status.
The Registered Nurse or Licensed Practical Nurse will ensure that all clients receive the highest quality of care which will enable them to remain in their home and in their community of choice.
*Responsibilities:*
· *Compliance*
o The RN or LPN will ensure compliance with all standards and guidelines established by the Louisiana State Board of Nursing (LSBN), The Louisiana Department of Health (LDH), and the agency.
· *Nursing Assessments*
o The RN or LPN will perform all client assessments within the guidelines of LSBN, LDH and the agency.
o The RN or LPN Assessment will determine if the agency is capable of providing appropriate services to meet the medical needs of potential and / or current clients.
o The RN or LPN Assessment will be performed on each client that is identified as in need of complex medical care and or non-complex medical needs delegation at least annually, and or following any and all hospital admits or ED visits.
o The RN or LPN Assessment will be used in the development of the client’s annual Comprehensive Plan of Care and revisions, if applicable.
· *RN or LPN Delegation*
o The RN or LPN will delegate, educate and monitor medication administration and other approved non-complex tasks to DSW’s, when applicable.

o The RN or LPN retains the right to refuse or withdraw delegation to the DSW based on the competency or documented disciplinary action(s) of the DSW.
· *Staff Training*
The RN or LPN will be responsible for initial and on-going medical trainings of all Administrative Staff and DSWs
o *Orientation*
· The RN or LPN will conduct basic medical training to all newly hired DSW’s
o *Medication administration and other non – complex task training*
The RN or LPN will conduct the 16 hour Medication Administration course (RN may be required to teach the 60 hour CMA Course once Certified, if Applicable)
· The RN or LPN will conduct other non-complex task training deemed appropriate as identified by client’s individual and specific needs.
· *Documentation*
o The RN or LPN will complete and provide the required documentation for client specific
Nursing /medical assessments and delegation within the guidelines of LSBN, LDH
and the agency.
o The RN or LPN will complete and provide the required documentation ensuring that the
DSW has met all training and competency requirements to adequately perform identified non-complex tasks including the administration of medications safely within the guidelines of LSBN, LDH and the agency.
o The RN or LPN will document all necessary aspects of client care and DSW interactions’ to ensure compliance with all guidelines of LSBN, LDH and the agency are followed.
o The RN or LPN will assess and assist the DSWs with the documentation of basic medical information that will allow proper assessment of client’s health status.

*Requirements:*
· Must have a valid Louisiana RN License or License Practical Nurse License.
· Have a minimum of at least one year related experience.
· Must be able to accept 24/7 on-call responsibilities to assure that the individuals needs are met and are of the highest quality attainable.
*Additional:*
· The ability to interact on a professional level with agency staff, State, local and regional entities, clients and their families, employees and representatives of other community-based services.
· Basic computer skills including Microsoft Office, Word and Excel.
· Must possess strong Administrative and Clinical skills.
· Be an effective Communicator and Educator.
